Hospital Discharge: Our clinical team works closely with commissioners, hospital staff and other professionals to ensure a safe and successful transition home for patients. We prepare our staff with the necessary training and competencies to meet the needs of each service user.
Nurse-led to Carer-led Care: Our approach to transitioning a package from nurse-led to carer-led focuses on building capability and confidence among family members and carers through educational sessions and hands-on training.
