Job Title: Senior Nurse Assessor

Contract Type: Initial 6-Month Contract – 37.5 Hours – Hybrid role
Day Rate: £240 – £280 per day

We are seeking a dedicated and skilled Senior CHC Nurse Assessor to join a Continuing Care and Learning Disability Team. This role supports children and young people (CYP) through comprehensive assessments and care transitions, ensuring they receive the best possible health outcomes.
Job Summary:
- The post holder will be responsible for facilitating multi-disciplinary assessments and child or young person continuing care reviews in line with standard operating policy and procedures. The post holder will adhere, at all times, to the relevant National Framework and guidance, NMC Code of Professional Conduct and work within established policies, procedures, protocols and systems ensuring quality standards and performance as directed by the All Age Continuing Health Care and Young Adults Lead
- The post holder will co-ordinate/facilitate multi-disciplinary team meetings including the child/young person/parent/carer where appropriate to ensure that a robust process is followed, providing an equitable decision regarding eligibility for continuing care.
Key Responsibilities:
- To facilitate multi-disciplinary assessments and child or young person reviews in line with standard operating policy and procedures.
- The post holder will be responsible for children and young people’s continuing care reviews and assessments of holistic nursing care needs for the child or young person who are to be placed or are currently in any primary/secondary care setting including social care and educational care settings.
- The post holder will be required to maintain constructive relationships with a broad range of internal and external stakeholders including social care, education services, child, young person, parent, or carer.
- To participate in 16– 18 years and 18 – 25 years (SEND) transition multi-agency meetings to review Continuing Care assessments, care plans and determine transitional arrangements with the multi-agency team, providing the consistency and predictability necessary to transition a child/young person’s care between care settings or levels of care within a care setting in a timely manner.
- To participate in joint commissioning for special educational needs and disability (SEND) focusing on the outcomes which make the most difference to the child or young person and their family, incorporating their views and aspirations to developing a holistic view of the child’s needs.
- Work with professionals to collaborate on an assessment of child’s needs, where they are complex.
- Lead multi-disciplinary assessments and reviews for CYP as per national frameworks and our standard operating procedures.
- Coordinate with a broad range of stakeholders including social care, education services, and healthcare providers to deliver holistic care plans.
- Manage complex case loads including safeguarding, Personal Health Budget (PHB) implementation, and transitions from children’s to adult services.
- Ensure adherence to all relevant healthcare regulations, maintaining the highest standards of care and compliance.
Requirements:
- Registered Nursing Qualification with NMC registration; paediatric qualification highly desirable.
- Demonstrable experience in children’s continuing care, SEND, and managing health service performance.
- Strong organisational and communication skills, capable of leading meetings and complex case management.
- Ability to work flexibly across various settings as required by service needs.
